All families are required to bring this packet; completed in its entirety, with the child on or before the first day of care provided. The packet must also be acompanied by a $50.00 registration fee and the child's up to date immunization records. If at any time you need assistance completing the form, or do not understand the form; please let the director know and we will be happy to assist you.  This packet is able to be printed right from the link below. If you do not have access to a printer, a hard copy can be obtained at The Learning Garden's addministrative office.
